After the revocation of an Olympic medal, are those who took lower places get upgraded?

Recently, there are numerous news about the Russian Olympic players being stripped of medals. There have been many similar cases in the past, too.

Once a sportsman gets stripped of his/her medal, do the others get "upgraded" automatically?
E.g., if the gold medal was stripped, does the player who took 2nd place automatically get "promoted" to the gold, the 3rd to the silver, etc.? If so, what is the process? Also, what happens to a sportsman who was the 4th and did not even appear on the podium?

If different committees/federations have different policies, let us confine the question to the Olympics and IOC only.
