Has a top seed tennis player been disqualified from a grand slam tournament before?
Based on the facts, it was not a default. That's all I can say. Every person or article stating that she was defaulted is incorrect.

It is wrong that Serena Williams has been defaulted at the 2009 US Open. She received a point penalty that led to her losing the match. Taken from Wikipedia: "The violation ended up as a point penalty due to receiving a warning for racquet abuse earlier in the match, thus ending the match in controversial fashion"

What happens if a tennis player challenges a decision for his opponent
No need to be a smartass ;) I meant that the specific case of a player challenging a call that is actually favourable to him is not dealt with. Normally such special circumstances are explicitly dealt with in the ATP rulebook. That's what I was referring to.

This case is not explicitly handled in the rulebook, but I can try to amend my answer.

Clearly he asking about the challenge system which is in place for all rule bodies. for instance, look it up on the ATP rulebook under "Electronic review".
